กรณีศึกษาที่สนใจ

Case : หญิงไทยคู่อายุ 39 ปี ภูมิลำเนา อ.ควนเนียง

Chief complaint : เจ็บคอ 2 วันก่อนมาโรงพยาบาล

Present illness : 2 วันก่อนมาโรงพยาบาล ผู้ป่วยมีไข้สูง ไม่หนาวสั่น ไม่มีน้ำมูก ไอเล็กน้อย มีเสมหะสีขาว รู้สึกเจ็บคอมาก กลืนได้น้อยลง เบื่ออาหาร เวลานอนสามีบอกว่ามีเสียงกรนอย่างที่ไม่เคยเป็นมาก่อน รับประทานยาลดไข้พาราเซตามอล อาการไม่ดีขึ้น จึงมาโรงพยาบาล

Past History: - ปฏิเสธประวัติโรคประจำตัว

- ปฏิเสธประวัติแพ้ยาแพ้อาหาร

Physical Examination

Vital sign : Body temperature 38.3 C, Heart rate 84 /min , Respiratory rate 22 /min ,Blood pressure 127/82 mmHg

GA : a Thai women, good consciousness

HEENT : mild injected pharynx,tonsil not enlargement,no exudates, no nasal congesion, no nasal discharge, no hearing loss by finger rub,

not pale , no jaundice, no lymphadenopathy

Cardiovascular system : normal S1S2 ,no murmur

Respiratory system : chest normal shape and movement, clear breath sound both lung

Abdomen : no distension, normal bowel sound, soft, not tender

Extremities : no rash, non pitting edema

Diagnosis : Streptococcal pharyngitis

ในผู้ป่วยสุขภาพดี อายุ 15 ปี ขึ้นไปที่รับการวินิจฉัยเป็น Streptococcal pharyngitis การใช้ The centor score จะมีความแม่นยำในการวินีจฉัยหรือไม่ เมื่อเทียบกับ Throat swab culture
