Case : เด็กหญิงไทย อายุ 6 ปี ภูมิลำเนา จ.ภูเก็ต
Chief complaint : มีตุ่มน้ำใสขึ้นตามข้อพับ มา 2 วัน PTA
Present illness : มารดาให้ประวัติว่า ในอดีต ตอนน้องอายุ 8 เดือน มีอาการผื่นแดง และ ตุ่มน้ำใสคัน บางครั้งมีลักษณะเป็นหนังแห้งกว่าปกติ เป็นขุย และเป็นสะเก็ดขึ้นที่จมูก แก้ม หน้าผาก ศีรษะ
ซึ่งมักจะขึ้นพร้อมกันทั้ง 2 ข้าง บางครั้งอาจลามไปที่ลำตัวตอนบน แขนขา และบริเวณที่สัมผัสผ้าอ้อมมักจะเป็น ๆ หาย ๆ เรื้อรัง ในรั้งนี้ จะขึ้นเป็นผื่นแดง มีตุ่มน้ำปน มีอาการคันมาก บริเวณที่น้อง เกาหรือถูมาก ๆ
มีน้ำเหลืองเยิ้ม และ หนังหนาขึ้นอย่างผิดปกติ จึงมาพบแพทย์
Past History : ปฏิเสธประวัติแพ้ยา , ปฏิเสธประวัติแพ้อาหาร
Family History : มีคนในครอบครัวเป็นโรคภูมิแพ้

Physical Examination

Vital sign : Body temperature 37.2 C Heart rate 65 /min
Respiratory rate 19 /min Blood pressure 120/80 mmHg
GA : a Thai girl, good consciousness, not pale ,no jaundice
HEENT : no conjunctivitis bulging and pale nasal turbinates with watery discharge on
nasal speculum exam
Cardiovascular system : normal S1 S2 , no murmur
Respiratory system : chest normal shape and movement ,clear breath sound both lung
Abdomen: no distension, normal bowel sound, soft, not tender,
Nervous system
Motor power: gradeV all
Sensory : intact
Normal reflex
Cerebellar sign : normal finger to nose test
skin : face head arms and legs have some group of tiny red spots (papules)
and blister-like lesions (vesicles) that enlarge and merge into a reddened,
itchy, sometimes
burning coin-shaped lesion two to 10 centimeters in diameter. The lesion may be
uniformly red at first, then clear in the center to form a "ring"


การรักษา
1. ทาด้วย steroid prednisolone
2. ให้ยาแก้แพ้ลดอาการคัน CPM
3. ให้ diazepam ก่อนนอน
