PH. ปฏิเสธโรคประจำตัวใดๆปฏิเสธการเข้ารับการผ่าตัดใดๆปฏิเสธประวัติแพยาแพ้อาหารใดๆปฏิเสธการสูบบุหรี่และดื่มเครื่องดื่ม alcohol
PE: V/S; BP 112/86 mmHg, PR 72 /min, RR 22/min, BT 36.8 C
GA: good consciousness, not pale
HEENT: not pale conjunctiva, no icteric sclera
Heart: normal S1S2, no murmur
Lung: clear both side, Rt=Lt, no adventitious sound
Abdomen: soft, not tender, no mass, normal bowl sound CVA negative
Ext: no edema, full ROM
Stress leg raising test -ve
Imp. Back pain
Rx. Diclofenac ( 25mg) 1 tab po tid pc # 20
Omeprazole (20 mg) 1 tab po ac # 10
Analgesic balm apply to the affect area for pain 3-4 time/day # I
