Chief complaint: แขนซ้ายไม่ขยับทันทีหลังคลอด
Present illness: ทารกเพศชาย น้ำหนัก 3,930 กรัม
คลอดด้วยวีธี Vacuum extraction เนื่องจากมี prolong second stage of labor (2hr 18min)
หลังจากคลอดศีรษะได้ เกิดShoulder dystocia แพทย์สามารถช่วยคลอดจนสำเร็จ
หลังคลอดสังเกตว่าทารกแขนข้างซ้ายไม่ขยับ ส่วนแขนข้างขวาขยับได้ปกติ สามารถกำมือได้ทั้ง2ข้าง
Maternal history: มารดาไม่มีโรคประจำตัว, ไม่มีความผิดปกติระหว่างการตั้งครรภ์, ฝากครรภ์ที่ รพ.หาดใหญ่ต่อเนื่อง เริ่มฝากครรภ์ตอน อายุครรภ์ 2เดือน
Physical examination:
GA- active, no dyspnea, not pale, no jaundice
HEENT- normocephalic, chignon, no cleft lip cleft palate
Thorax- no subcostal or substernal retraction, normal breast sound, lung clear,
Heart-normal S1S2, no murmur,
Abdomen- no scaphoid abdomen, midline umbilicus, A:V=2:1
Anus-patent
Extremities-decrease movement of left arm, no crepitation on left shoulder
Reflex: Moro negative left arm
Impression: Erb’s palsy
Patient: Newborn with brachial plexus injury
Intervention: Time
Comparison: None
Outcome: Self resolved of brachial plexus injury
Question: What number of newborns who have brachial plexus injury can self resolved at 6month, 1years and 3 years?

เป็นเรื่องที่น่าสนใจนะ เราไม่เคยรู้มาก่อนเลยว่า brachial plexus injury มี self resolved ด้วยจนเธอบอกนี่แหละ อีกหน่อยเจอเด็กที่มีความผิดปกติแบบนี้จะได้ไม่ต้องตกใจมาก^_^
