หญิงไทยคู่ อายุ 25 ปี
CC: ตกขาวผิดปกติ 3 สัปดาห์ก่อนมาโรงพยาบาล
PI: 3 สัปดาห์ก่อนมาโรงพยาบาล ผู้ป่วยมีตกขาวผิดปกติ ลักษณะขาวเป็นก้อนคล้ายแป้ง ปริมาณมากติดกางเกงในทุกวัน ไม่มีฟอง ไม่มีกลิ่นเหม็น มีอาการคันบริเวณช่องคลอดและอวัยวะสืบพันธ์ภายนอกร่วมด้วย ปัสสาวะไม่แสบขัด ผู้ป่วยสังเกตเห็นผื่นแดงรอบๆอวัยวะเพศ
PH: ปฏิเสธโรคประจำตัว DM,HT
ปฏิเสธการแพ้ยาแพ้อาหาร
OB-GYN: ปฎิเสธปฎิเสธโรคติดต่อทางเพศสัมพันธ์
ไม่เคยมีอาการเช่นนี้มาก่อน
คุมกำเนิดโดยยาเม็ดคุมกำเนิด
PE: V/S BT 37.2 C, BP 120/80 mmHg, RR 20, PR 80
GA:Good conciousness
HEENT: not pale, no jaundice
HEART* LUNG: WNL
ABDOMEN: soft, not tender
PV:
NIUB: มีผื่นรอยแดง และรอยเกาที่อวัยวะสืบพันธ์ภายนอก
Vg: curd like d/c
Cx: normal
Ut: normal size
Adn: not tender
Culdesac: no bulging
INVESTIGATION
Wet smear: budding yeast and pseudohyphae
Dx: cadida vulvovaginitis
Tx: Clotrimazole 100 mg สอดช่องคลอด วันละ 2 เม็ด ก่อนนอน 3 วัน
Clotrimazole 1%cream 5กรัม ทาช่องคลอด 7 วัน
ปัญหา การรักษา cadida vulvovaginitis โดยใช้ยาสอดช่องคลอดและยาทาตาม case นี้ เมื่อเปรียบเทียบกับการใช้ยารับประทาน(ketoconazole 200mg รับประทานวันละ 2 ครั้ง 5วัน)ร่วมกับทา อย่างไหนให้ผลดีในการรักษามากกว่ากัน(หายเร็วกว่ากัน)

search หาคำว่า candidal vulvovaginitis clotrimazole oral vaginal suppository
ก็จะได้มา 2งานวิจัย มีอยู่ 1งานวิจัยที่สามารถตอบคำถามได้

Analysis of the results of doctor and patient assessment showed that both treatments were highly effective in reducing the signs and symptoms of infection.
Mycological cure rates were similar for both ketoconazole (82%) and clotrimazole (86%). The corresponding mycological relapse rates were 0% and 18%.

เข้ามาแจมเฉยๆได้มะ อ่านของโบว์แล้ว คือ มันไม่ต่างในผลการรักษาเท่าไหร่ แต่ถ้าเป็น local ดีกว่านิดนึง แต่มี relaspe มากกว่าใช่ป่าว แต่เราว่าถ้าเอามาใช้จิงนี่ กินดีกว่ามั๊ย ใช้ง่ายกว่า ยกเว้นกรณีมี contraindication เนอะ และถ้าในความจริงใช้ยาเหน็บน่าจะมีผลการรักษาที่ลดลงอันเกิดจากการใช้ยาไม่ถูกต้องด้วย (มั๊ง)
