CC : ท้องโตมากขึ้น 1 สัปดาห์ก่อนมาโรงพยาบาล

PI : 1 สัปดาห์ PTA มารดาสังเกตว่าผู้ป่วยมีท้องโตมากขึ้นอย่างรวดเร็ว อาการอื่นๆปกติ
รับการรักษาที่รพ.ตากใบ ผล ultrasound : multiseptate cystic with solid 30 cm diameter

Past history : ไม่มีประวัติเคยได้รับการกระทบกระแทกหรือเกิดอุบัติเหตุบริเวณอุ้งเชิงกรานหรือ
ช่องท้องมาก่อน

Family history: ไม่มีประวัติโรคร้ายแรงทางพันธุกรรม เช่นโรคเลือดหรือโรคมะเร็งในครอบครัว

HEENT no thyroid gland enlargement
Lymph nodes preauricular, postauricular, submandible, submaxillar, cervical, supraclavicular lymph node cannot be palpated.
Breast No mass, no discharge, no retraction, no skin lesion and no axillary lymph nodes enlargement

Cardiovascular system Normal
Abdomen mild distension, not tender, no palpable mass, ascites negative by shifting dullness, well define abdominopelvic cystic mass 40 cm in diameter, movable, smooth surface, well define border, no hepatosplenomegaly, abdominal circumference = 87 cm, normal active bowel sound at peripheral
PV MIUB: atrophic
Vag: atrophic
Cx: os close, small size, no lesion, point posterior
Uterus + adnexa: complexed mass roughly 12 cm, mostly deviated to left pelvic side wall, hard consistency, fixed, not tender.
CDS: tumor bulging in CDS
RV-exam: smooth rectal mucosa, irregular nodule of US ligament (not anterior rectum.)

LDH & B HCG & AFP : normal

ประเด็นปัญหา ในผู้ป่วยรายนี้ สงสัยว่า น่าจะเป็น ovarian tumor ที่เป็น malignancy การทำ peritoneal dialysis สามารถช่วยวินิจฉัย ovarian cancer ได้หรือไม่
