CC: หอบเหนื่อยตั้งแต่แรกเกิด
PI: G1P0 GA 33 wk by Ballard score NL น้ำหนักแรกคลอด 1700 g คลอดปกติไม่มีปัญหาแทรกซ้อนระหว่างคลอด หลังคลอดมีอาการหาบใจเร็ว หอบเหนื่อย ซี่โครงบาน ไม่ซีด ไม่เขียว ไม่มีความผิดปกติส่วนอื่นของร่างกาย
PE :vital sign BT 37 C RR 68 /min HR 130/min
GA: a thai male newborn,no cyanosis
HEENT: anterior fontanelle 2x2 cm,posterior fontanelle 1x1cm,no bulging,normal shape,no caput,no cephalhematoma,no nasal septum deviation,nasal flaring
Heart: continous machine murmur
Lung: crepitation both lung, subcostal retraction
Abdomen: soft,no distention,no mass,umbilical at midline A:V=2:1
Genitalia&anus:patency anus,male genitalia
Extremities:no deformitites
แพทย์ทำการส่งCXR ผล- ground-grass appearance both lung,hypoaeration
Imp: 1. RDS
2.R/O PDA
ทารกมีอาการหายใจเหนื่อย แพทย์จึงรับไว้รักษาในNICU ต่อมาวันที่7 อาการดีขึ้น หายใจหอบเหนื่อยน้อยลง ไม่มีซี่โครงบาน แพทย์จึงส่ง echocardiogram พบว่าเป็นPDA ขนาด 3mm แพทย์ได้วางแผนการรักษาต่อ โดยการให้ indomethacin ช่วยในการปิดductus arteriosus แพทย์อีกท่านกล่าวว่า ibuprofen ก็ช่วยในการปิดductus arteriosus ได้เช่นกัน
ปัญหา ibuprofen มีประสิทธิภาพในการปิดPDA ได้เทียบเท่ากับ indomethacin หรือไม่ ทั้งผลในการปิดPDA และผลข้างเคียงที่ตามมา

แพรค่ะ ลองอ่านดูนะค่ะ เป็นการเทียบยา ibuprofen กับยาอื่นๆที่ใช้ใน preterm and/or low birth weight infants เพื่อปิด PDA อ่านคร่าวๆเค้าบอกว่าไม่ต่างกัน (เน้นอ่านคร่าวๆ) เราเอามาจาก Cochrane เค้าบอกว่า
....No statistically significant difference in the effectiveness of ibuprofen compared to indomethacin in closing a PDA was found. Ibuprofen compared with indomethacin reduces the risk of oliguria and is associated with lower serum creatinine levels following treatment. Pulmonary hypertension has been observed in three infants after prophylactic use of ibuprofen and one infant receiving ibuprofen for treatment in this review developed pulmonary hypertension. One additional case of pulmonary hypertension following treatment with ibuprofen to close a PDA was identified from the literature. The available data support the use of either drug for the treatment of a PDA. As both drugs are equally effective in closing a PDA, the clinician needs to weigh the potential side effects of one drug vs. the other when making a decision which drug to use. The most urgent research question to be answered is whether ibuprofen compared to indomethacin confers an improved rate of intact survival (survival without impairment) at 18 months corrected age and at the age of school entry.
